On 8 September 2016, the Office of the Secretary of Defense directed a new uniform, Tri-Service Periodic Health Assessment (PHA) with reference (a). Reference (b) was published to then provide additional implementation details for the new Department of Defense (DoD) PHA. The DoD PHA is designed for electronic completion only. This ALNAV provides instructions for Service Members and medical personnel to complete the new PHA. 2. All Department of the Navy personnel must complete the new DoD PHA, DD Form 3024, electronically. DD Form 3024 includes the required annual mental health assessment (MHA) and requires a person-to-person review of the MHA with a provider per the 2015 National Defense Authorization Act (NDAA). Maintaining medical readiness is every Service Members responsibility; the PHA is intended to ensure our Service Members are fit for full duty and medically ready for both deployment and overseas assignment. For this reason, it is vital that any duty or deployment limiting conditions are noted and addressed appropriately, as discussed in reference (c). 3. The new electronic DoD PHA (ePHA) is located at the ePHA website https://data.nmcphc.med.navy.mil/pha/Index.aspx. This site can be accessed from a home or government-owned computer. A common access card (CAC) is recommended but not required to access the system. To create an account using a username and password rather than a CAC, Service Members are required to supply both their DoD Identification and social security numbers. 4. The new DoD PHA is a Four-Step Process: a. Step One: The Service Member completes the self-assessment online at the website listed in paragraph 3. b. Step Two: A medical representative will conduct a record review. c. Step Three: An approved, privileged provider or Independent Duty Corpsman (IDC) will discuss the MHA section of the DoD PHA with the Service Member, either in person, by telephone, or by video teleconference. d. Step Four: An approved, privileged provider, or IDC will conduct a final review of the Service Members PHA. 5. PHA Implementation and Completion Alternatives. Bureau of Medicine and Surgery (BUMED) and the Navy and Marine Corps Public Health Center (NMCPHC) are aware some units (e.g. operational units) were not able to meet the DoD- mandated implementation deadline of 31 December 2017 due to information technology (IT) challenges. Until the non-internet-based solution developed by NMCPHC is fully distributed, those units that continue to have IT challenges may continue to conduct the paper PHA using the NAVMED 6120/4 (Rev. 03/2008) and must complete a MHA, DD Form 2978 (can be conducted in the Electronic Deployment Health Assessment (EDHA) module at https://data.nmcphc.med.navy.mil/EDHA/ in order to comply with the 2015 NDAA-mandated annual MHA. a. For ships and units unable to access either the ePHA or EDHA websites, the store and forward version of the stand-alone MHA, DD Form 2978, produced by NMCPHC for use without internet access, is recommended to comply with the annual MHA requirement and will not require printing of the MHA. This version of the MHA, along with instructions on loading and using the program, can be obtained by contacting the NMCPHC at usn.hampton- roads.navmcpubhlthcenpors.list.nmcphc-edhahelpdesk@mail.mil. b. If the ePHA and EDHA websites, or the DoD PHA and EDHA store and forward versions are not currently available, a paper copy of the MHA, DD Form 2978, must be completed manually. The completed MHA form can be scanned and sent in a secure manner to the NMCPHC helpdesk via e-mail at: usn.hampton-roads.navmcpubhlthcenpors.list.nmcphc-edhahelpdesk@mail.mil. Alternatively, a hard copy can be mailed to: Navy and Marine Corps Public Health Center, ATTN: PHA Helpdesk, Mr. Azad Alkoshnaw, 620 John Paul Jones Circle, Suite 1100, Portsmouth, VA 23708. A hard copy of the MHA should then be placed in the Service Members service treatment record or scanned and placed in Theater Medical Information Program-Maritime (TMIP-M), Armed Forces Health Longitudinal Technology Application-Theater (AHLTA-T), or AHLTA. c. All MHAs completed outside the DoD PHA process still require a person-to-person review. d. Once the non-internet-based solution for the DoD PHA is fully distributed, available for use, and functional for all units that have not been able to comply with the new DoD PHA requirement due to IT and internet connection challenges, use of the NAVMED 6120/4 to complete the PHA will no longer be authorized. BUMED will track progress of distribution of this solution and submit further guidance when NAVMED 6120/4 is no longer authorized. 6. Readiness Reporting a. For Navy, Marine Corps, and Coast Guard personnel, a Service Members Individual Medical Readiness status will be automatically updated in Medical Readiness Reporting System (MRRS) for all PHAs completed on the ePHA website. b. For PHAs completed with the NAVMED 6120/4 and DD Form 2978, MRRS must be manually updated once both components are complete and reviewed by an authorized provider. c. Shipboard units completing the PHA with the NAVMED 6120/4 and DD Form 2978 will enter completion data into TMIP-M to be automatically uploaded to MRRS when available. d. MRRS is automatically updated for Navy and Marine Corps personnel completing the PHA process at an Air Force or Army medical treatment facility when the Service Member uses the ePHA website to initiate and complete the self-assessment (Step One) of the DoD PHA. The PHA will not be considered complete until all steps have been completed and signed by a PHA provider. 7. Additional information for completing the DoD PHA is on the Navy ePHA website: https://data.nmcphc.med.navy.mil/pha/Index.aspx. Information posted on the website contains the most up-to-date guidance for completing the DoD PHA. Topics will be updated as necessary and initially contains: a. Guidance for all Service Members completing the DoD PHA. b. Guidance for Service Members completing the DoD PHA at non -Navy MTFs. c. Guidance for Service Members stationed remotely completing the DoD PHA. d. Guidance for healthcare personnel reviewing PHAs. e. Guidance for documenting and coding PHAs. 8.
