- "Freedom of Information Act (FOIA) Requests for DD Form 2910

If an Active-Duty victim would like another copy of their DD 2910, they can contact their SARC to request that copy.
If a victim is requesting a copy of their DD 2910 and is no longer active duty the victim has the following two options:
A Veteran may utilize the assistance of the Department of Veterans Affairs (VA). As a third party requester, the VA follows certain procedures in order to request copies of forms and reports through the respective military services for VA claims. In order for the VA to obtain a copy of a DD Form 2910 and/or reports from the Navy, the VA submits a VA Form 21-4142, Authorization to Disclose Information to the Department of Veterans Affairs (VA), signed by the Veteran, accompanied by the VA's letter officially requesting the DD Form 2910 or other specified reports/documentation. The VA may email the CNIC HQ SAPR office at CNIC_HQ_SAPR@navy.mil to request a secure DoD SAFE submission or submit the request through the FOIA-Online tracking and management web-based tool.
- Online https://foiaonline.gov/foiaonline/action/public/home and submit your FOIA request for documents. Once you have submitted the appropriate request in FOIA Online, you will be contacted if any additional information is needed to process your request.

If the Victim would like to request a copy of the DD Form 2910 Victim Reporting Preference Statement and/or other relative reports and documents without the help of the VA, the Victim may submit a written request through the Freedom of Information Act (FOIA). All FOIA requests for documents submitted to the US Navy must be processed through the FOIA-Online tracking and management web-based tool. Please go to:
- Online https://foiaonline.gov/foiaonline/action/public/home and submit your FOIA request for documents. Once you have submitted the appropriate request in FOIA Online, you will be contacted if any additional information is needed to process your request.

There have been some issues with NMCI computers and Adobe forms, the In-browser version of acrobat won’t open forms. To resolve this issue, right-click on the document link and download it to your desktop. Then you can open it with Acrobat Reader. An info bar, will appear and you can click on “Enable All Features” to open the form.
Additionally, Both the SADR and SAIRO forms can also be found at http://forms.documentservices.dla.mil/ once you are on the site, go to the bottom and click on forms. At the top in search criteria put in the stock number. The SAIRO is NONSN00013941 and the SADR is NONSN00014407. When the form comes up click on the disk icon, the "adobe please wait" screen may appear, right click on the document and click "save as", to download it to your desktop, do not click open when done downloading. Close out and go to your desktop and open the file with Adobe Reader, this should allow the "Enable All Features" button.
