Survey Approvals

 
Survey approval requirements vary depending on the nature and scope of the proposed survey, number and type of personnel to be used as participants, survey topic, and intended purpose/usage of results all influence the documentation that must be completed. 

The Navy Survey Office can only authorize surveys focusing on Navy Personnel (Active Duty, Reserve, Civilian). For surveys of personnel from multiple branches, the Office of People Analytics will have the ultimate approval authority. Surveys involving contractors and/or surveys that ask about sensitive topic issues (e.g., suicide, sexual assault, racial equity, gender relations, COVID, climate change) will need to be approved by the Office of Management and Budget.

Documentation required for approval of Navy Surveys may include any of the following items: 
Survey Request Form. This form begins the survey approval process and will provide the Survey Program Office with information to determine which of the requirements below may be required for each survey. 

Example Survey Request Letter/Action Memo.  The action memo serves as a cover sheet for your survey proposal. It should cover the essential information of who you want to survey, why, when, how you plan to conduct the survey, and what you plan to do with the results.

Example Survey Endorsement Letter.  Surveys that are conducted from multiple commands or are not part of the conduct of official Navy work may require the endorsement of a Flag or Flag equivalent sponsor.

Report Analysis Data. This form provides an estimate for how much Sailor time will be expended during the conduct of your survey. In order to generate the estimate, you may use the CAPE tool (CAC required).

Final Survey Draft. Your final survey draft should be included..
 
Tools for Data Collection
Before selecting any commercially available survey tool, you should check to make sure that it meets all relevant DoD and Navy Information Assurance and IT security requirements (e.g., FedRAMP). Since the data will be stored on commercial servers there is increased risk of harm or embarrassment if the data are somehow compromised. The Navy Survey Program Office has access to tools that can help with data collection efforts and are currently the only set of approved tools for use. Please contact the Survey Program Office at navy.surveys.fct@navy.mil in order to request access to available tools.
