Conditional Release

 
Requesting Transfer
Reserve enlisted desiring to transfer to another service must have the gaining service submit a Conditional Release request (DD Form 368) to Navy Personnel Command (PERS-913) for approval. 

If the member is attached to/drilling with a Reserve Command, then a Command endorsement is also required.  Use standard Navy letter format for the endorsement.

NRC and Unit Commanding Officers are not authorized to sign the DD Form-368. The only signatures authorized are the members, recruiters and PERS-9 approving authority.
 
Intel Community Requirements
Enlisted in the Intel community are also required to have an endorsement from Information Dominance Corp Reserve Command.
Members in the Intel community must have endorsements from the Unit, NRC, Region NAVIFORES and Force NAVIFORES. That will be four endorsements before it is sent to PERS-913.
