EFMP enrollment is mandatory and required immediately upon identification of a medical, mental health and/or educational need for any dependent documents on a services members Record of Emergency Data and Dependency Application (RED/DA /Page 2)
Please use DD Form 2792, Exceptional Family Member Medical and Educational Summary and DD Form 2792-1, Exceptional Family Member Special Education/Early Intervention Summary (required if dependent children 3-18 (23 if enrolled in full time school) years of age has an active IEP/ISFP.) to complete enrollment submission. ***There is a known problem with Adobe for DoD fillable forms and Navy web systems. You'll need to save the form to your desktop (right-click on the link to the file and select "Save Link As"), then open the form with Acrobat Reader and then click "Enable All Features" in the yellow info bar to view the forms. ***
Every Navy Military Treatment Facility (MTF) appoints an EFMP Coordinator to facilitate the enrollment process. Coordinators provide enrollment forms, offer help in the preparation of the forms, review completed forms for accuracy, and forward the enrollment package to the Central Screening Committee (CSC) via NFAAS. If you are not located near an MTF, forms may be available from one of the following locations:

Fleet and Family Support Centers (FFSC) Personnel Support Offices

Navy Liaison Offices

Navy Personnel Command (NPC) EFM Program Managers(PERS-456) in Millington, Tennessee
Return the completed forms to the MTF. The Coordinator will review and sign the forms and forward them to a CSC.
Review the forms before signing. You are responsible for their completeness and accuracy.
An official at your child’s school must complete Addendum B Special Education.
The treating physician(s) should complete the medical forms.
In areas with no MTF or EFMP Coordinator, you can email the EFMP application forms (with other required documentation attached) to efmp_admin.fct@navy.mil.
CENTRAL SCREENING COMMITTEE (CSC)
Effective 01 October 2024, the CSC transitioned from the Bureau of Medicine (BUMED) Force Medical Readiness (N10D) to the single-lead EFMP office (NAVPERSCOM (PERS-456)).
The CSC is a medical review committee consisting of healthcare professionals responsible for reviewing completed EFM enrollment packages. The CSC makes EFMP enrollment determinations and adjudications on Level of Needs and priority placement on the Navy Housing waitlist.
