When members receive their Notice of Eligibility (NOE) they have 90 days from the date of their NOE to submit the DD FORM 2656-5 or equivalent to PERS-912 or they are subject to default laws. If your NOE was issued before 12/31/2000, the default was for No Reserve Component annuity. If your NOE was issued after 01/01/2001, the default is for full, immediate annuity for all dependents you had at that time. Approximately 90% of members default. If you submitted the DD FORM 2656-5, a copy should be in your OMPF and will be pulled by PERS-912. Dependent children fall off the annuity at age 22. You may leave blocks 35-39 blank for full research on receipt of your forms. If additional information is required, you will be contacted by mail, email and/or phone. If members default into coverage but do not want the coverage they can:
Request Administrative Correction within one year of date of default.
File a request (DD149) with the Board of Corrections for Naval Records to have the coverage removed. Member must be able to prove they did not receive their NOE or any information about Survivor Benefits.
DFAS offers a one-year OPT-OUT period for members between the 25th and 35th month of receiving retired pay. SBP fees will stop, RCSBP fees will continue, and no refunds will be given for fees already paid.
RCSBP and SBP combined cost = approximately 10% of the member’s monthly retired pay. The RCSBP portion of the fee is approximately 3.5%. Following the member’s death and upon application, the beneficiary would receive 55% of the member’s retired pay.

Spouse’s signature is only required if RCSBP coverage was properly declined within 90 days of receipt or their NOE AND member and spouse wish to decline SBP OR they are electing coverage for a reduced amount (based on anything less than full gross pay). Spouse must sign in front of a notary. The spouse cannot sign on a date before the member. The spouse can sign on the same day or any day after the member signs.

If you are transferring to a retired pay status from a drilling status (DSC 200), you must submit your Retirement with Pay via NSIPS. Current policy requires endorsements from your chain of command. From Main Menu of NSIPS -> Employee Self Service -> Retirements and Separations -> Request Retirement/Separation Under Request Details, Choose Request Type “Non-Regular Retirement with Pay”. Upload your DD108 and DD2656 to the “Attach/Comment/Recommend” section Your request will route for approval to your command, followed by your NRC and lastly to NPC (PERS-912). You will need to monitor the routing to ensure your request is received by NPC within 180 days or it will be automatically denied. Once at NPC the Approval Status will remain as "Pending" until the day after your effective date. Once PERS-912 has determined that you are eligible, we will notate this approval in the "Comments and Recommendations" section of your NSIPS request. Once your effective date has passed, your point record will be closed out, retirement approved and DSC will change to 900. Retirement orders with final points will then be issued and your retirement documents will be forwarded to DFAS for final setup of your retired pay account. Processing at DFAS may take 30+ days. You can call DFAS at 1-800-321-1080. You may participate up to the day before your retirement effective date, but please let PERS-912 know your last participation so we can ensure all your retirement points are credited prior to closing your record. It can take up to two weeks for drills, AT/ADT or active duty (please provide your DD-214 if on extended active-duty orders prior to retirement) points to populate in your record. You can enter a comment in your NSIPS retirement request in the Comments section with the dates of any participation that is not yet reflected in your point record or call MNCC at 1-833-330-6622. Please note that if you participate up to the day before your retirement, your initial retirement pay will be delayed, but you will be paid retroactively.

Retirees under the age of 60 may be eligible to purchase Tricare Retired Reserve. Retired Reserve ages 60-64 are eligible for TRICARE Select and TRICARE Prime where available and if you become Medicare-eligible due to disability, you may transition to TRICARE For Life as early as age 60. Retired Reserve Age 65 and above begin TRICARE For Life and you must enroll in Medicare Part B. For further information, visit any of the links provided.

For those who entered military service after 7 September 1980, the base pay is calculated by adding the base pay available for the member (commensurate with their rank and longevity) for each of the 36 preceding months prior to drawing retired pay and dividing that by 36 to get an average pay rate for the final 3 years. Note that for purposes of entering the pay tables, a member’s longevity starts with the pay entry base date (PEBD) and continues to accrue as long as the member holds Retired status until the member starts to draw retired pay. Because of this standard, most reserve members will max out on the longevity scales by the time they reach Retired Pay Eligibility Date.
